Handy with a saw.
The Surgeon is a post prestige NPC that unlocks the ability to spawn the Robo Chicken, The Cursed, The Colossus, and The Infernal. He can be unlocked by using 50 
Astro Coins to form the first Rift Item and then another 25 
Energy Cubes to open the Rift and then summon him.
He is a post prestige NPC and with him the player can unlock new Legendaries.
The surgeon offers unlocking 4 additional Legendary creatures (it is a one-off cost):
Robo Chicken for 50 
Astro Coins
The Cursed for 150 Astro Coins
The Colossus for 200 Astro Coins and 50 
Energy Cubes
The Infernal for 300 Astro Coins and 150 Energy Cubes
